Appointments

Visit Checklist

  • Valid photo ID
  • Insurance card(s) (if applicable)
  • All radiology reports and imaging (ultrasounds, MRIs, and CT scans)
  • List of medications (if any)
  • Any other relevant information

**Valid photo identification (ID) and your insurance card(s) are required for your visit. Without current insurance information, you will need to reschedule your appointment.

Appointment Cancellations

Please call the office during business hours and allow at least a one-day notice so we can offer your appointment to other patients. Cancellations may also be made through the patient portal.  We recognize your time is valuable and make every effort to run on time. Occasionally, emergencies or patients require a little more time, and these may cause scheduling delays beyond our control. We apologize if we keep you waiting.

Late Policy

Please contact the office if you will be late and we will do our best to accommodate you. Arriving late may mean that you will have to wait until after the patients in their scheduled time slots are seen first.

No-Show Policy

We reserve the right to withhold appointments after two no-show visits.
